It sounds like the level tried to save, but failed.
The failed save might be why the level builder didn't create backups, but I'm not sure.
With the .mmlv level file being blank, and no level builder backups, I'm not sure if the level is recoverable.

From my own personal experiences, N/A levels only appear in offline play if MMM is trying to access a level file that either doesn't exist (file is deleted or moved while MMM is trying to access it), or can't be read (data corruption, saving glitches, or poor text-editing). Probably a dumb idea and hard to implement, but what about On/Off Water?

Hit an On/Off Switch and the On/Off Water drains from the area, hit the switch again to fill it back up.

It could lead to some neat water gravity manipulation puzzles.
