Mega Man's main problem in the past was that games lacked evolution, and they where pushing out games too fast. It sounds like the level tried to save, but failed.
The failed save might be why the level builder didn't create backups, but I'm not sure.
With the .mmlv level file being blank, and no level builder backups, I'm not sure if the level is recoverable.

From my own personal experiences, N/A levels only appear in offline play if MMM is trying to access a level file that either doesn't exist (file is deleted or moved while MMM is trying to access it), or can't be read (data corruption, saving glitches, or poor text-editing).
